Liečba generalizovanej agresívnej parodontitídy u pacientky s viacerými systémovými rizikovými faktormi

Cieľom práce je poukázať na dôležitosť diagnostiky a liečby parodontitídy podľa aktuálnych postupov u pacientky s reumatoidnou artritídou (RA), obezitou a ochorením diabetes mellitus. Po dôkladne odobratej anamnéze, klinickom vyšetrení tkanív parodontu a RTG vyšetrení bola stanovená diagnóza agresívna parodontitída. Pacientke bol vypracovaný individuálny liečebný plán podľa súčasných vedeckých poznatkov v medicíne založenej na dôkazoch - "evidence based dentistry". Okamžite po subgingiválnom ošetrení bola pacientke indikovaná systémová antibiotická liečba skladajúca sa z amoxicilínu v kombinácii s metronidazolom ako zlatý štandard antibiotickej terapie v liečbe parodontitíd. Počas reevaluácie sme pozorovali pokles hĺbky sondáže, zisk klinického attachmentu a pokles hodnôt indexu krvácania po sondovaní BOP. Po ukončení aktívnej parodontálnej liečby boli miesta s hĺbkou sondáže 5 mm vyžadujúce zvýšenú pozornosť iba 3, pričom všetky s negatívnym výsledkom BOP.

Treatment of generalized aggressive periodontitis in a patient with multiple systemic risk factors

This study aims to point out the importance of diagnosis and treatment of periodontitis according to the latest methods in a patient with rheumatoid arthritis (RA), obesity, and diabetes mellitus. After a thorough history, clinical examination of the periodontal tissue, and X-ray examination, a diagnosis of aggressive periodontitis was established. An individual treatment plan was developed for the patient according to current scientific knowledge in evidence-based medicine. Immediately after subgingival treatment, the patient was indicated for systemic antibiotic therapy consisting of amoxicillin in combination with metronidazole as the gold standard of antibiotic therapy in the treatment of periodontitis. During the re-evaluation, we observed a decrease in probing depth, clinical attachment gain, and a decrease in bleeding on probing (BOP). After completing the active periodontal treatment, there were only three sites with a depth of 5 mm requiring increased attention, all of them with a negative BOP result.
