Anesthesia for the mental foramen is one of the standard methods in dentistry. It is a simple anesthesia, but it depends on the exact injection to the mental foramen. The aim of the study was to determine variations in the horizontal localization of mental foramen. The location was identified on OPG images. The main criterion for assessing the location was the relationship of mental foramen to the tips of the roots of the lower premolars /1.PM and 2.PM/and the 1st molar /1.M/. 5 localizations of foramen mentale were determined. Localization between radix of 1.PM and 2.PM, localization under radix of 2.PM, localization under radix of 1.PM, localization between 2.PM and mesial radix of 1.M, localization under mesial radix of 1.M. A total of 290 OPG images were evaluated, 149 men and 131 women on the right and left side. Average position of mental foamen of all investigated groups was as follows: 1/ between the radix of the 1st and 2nd premolar - 51% to 68%, 2/ under the radix of the 1st premolar - 2% to 4%, 3/ under the radix of the 2nd premolar - 20% to 33 %, 4/ under the radix of the 2nd premolar and the mesial radix of the 1st molar - 4% to 6%, 5/ under the mesial radix of the 1st molar there were only 2 cases. Compared to the observations of other authors, the results of this study are close in most parameters. They are most correlated with the observations of the authors Zmyslowska-Polakovska et al., who indicate the position between the 1st and 2nd PM at 53% to 60% and below the 2nd PM at 30% to 38%. from which it follows that the horizontal localization of the mental foramen has a relatively wide range, therefore before each anesthesia to the mental foramen and with every surgical intervention in the area of the body of the mandible (tooth extraction, root tip resection, cystectomy, implantation, etc.) it is necessary to verify its exact location on the OPG image. It is also advisable to note the vertical position of the mental foramen.
