Ultrasound Guided Suprazygomatic block of the sphenopalatinal ganglion and its use as method of anesthesia in surgical treatment of odontogenic maxillary sinusitis

ABSTRACT Performing surgical treatment of chronic sinusitis or antral cysts is not that extensive procedure, however in local anesthesia it can be painful due to extensive inflammation of sinus mucosa, because of that we usually use general anesthesia to undergo the surgery. General anesthesia is overall safe and those with significant health conditions can undergo procedures under it, but it can also have a lot of complications. Nowadays due to discovery of modern techniques , monitoring equipment and safe anesthetic agents, there is decline in morbidity and mortality rates, however the incidence of minor and more common complications has not change significantly. The most common postoperative complications after surgery in orofacial area performed in general anesthesia are sore throat, nausea, vomiting, psychological changes in different forms, respiratory difficulty or hypertension. This is why we try to use an alternative form of anesthesia in preforming this kind of surgeries. The use of the sphenopalatine ganglion block has been proposed as a treatment option for a variety of pain syndroms such as cluster headache, trigeminal neuralgia, migraine headache, or to reduce the need for anesthetics after endoscopic sinus surgery. In this article we want to present our experiences with using blockage of the sphenopalatine ganglion as an alternative to general anesthesia in surgical treatment of chronic sinusitis or in removing of antral cysts.
