Chronic Nonbacterial Osteomyelitis of the Mandible in a 9-Year-Old Patient: A Case Report

Chronic nonbacterial osteomyelitis (CNO) of the mandible is a rare autoinflammatory disease primarily affecting children and adolescents. Prolonged diagnostic delay — typically 11–15 months — results from a nonspecific clinical picture mimicking bacterial infection, in the absence of fever, positive cultures, and significant inflammatory laboratory markers. We present a case of a 9-year-old female patient with recurrent inflammatory involvement of the mandible. After several months of unsuccessful antibiotic management, CNO was histologically verified. Retrospective scoring according to the EULAR/ACR 2025 classification criteria reached 91 points (threshold ≥55), unequivocally confirming the diagnosis. Treatment with pamidronate combined with rheumatological follow-up led to progressive bone healing. The case highlights the need for a multidisciplinary approach and the inclusion of CNO in the differential diagnosis of inflammatory jaw diseases in children.
