Impacting or dislocation of a fractured tooth root or of its part into the maxillary sinus is a rare complication during tooth extraction in the premolar and molar regions of the maxilla. Clinical studies report that these complications occur in 0.02-0.1% of all tooth extractions in the upper jaw. Most often occurs dislocation of one root, very rare dislocation of several roots and their parts into the maxillary sinus occur during extraction of the first molars, followed by the second and third molars and premolars. Rarely this situation can occur during extraction of upper canines in patients with large and irregular sinus maxillaris. This complication can cause severe problems in a form of improper oroantral communication, fistulation, chronic sinusitis and maxillary empyema, especially in cases with delayed treatment. In the years 2015-2016, 25 patients examined at the Department of Oral and Maxillofacial Surgery of the University Hospital Bratislava, were diagnosed radix in antro. In 24 patients, there was a unilateral affliction, in 1 patients the radix in antro occurred bilaterally. The aim of this paper is to point out the etiology of this complication during pulling out the tooth, the diagnosis options and adequate therapy procedures.
