Blade implants and their modifications as an unconventional possibility of implantation rehabilitation of the atrophic mandibular ridge. Part 1.

Blade implants (BI), although to a limited extent, find application in the implantation procedures of significantly resorbed alveolar ridge in distal sections of the mandible. They are used in situations where the use of enosseal cylindrical implants has reached the limits of application possibilities and the application of osteotomy or augmentation procedures does not bring any long-term prognostic success. The clinical part of the paper deals with a rational application technique of blade implants. In the experimental part, using FEM analysis, the authors evaluate the influence of masticatory forces on the body of the blade implant and the surrounding bone. The results show a significant concentration of stress in the neck of the implant and in the surrounding compact bone. The stress levels in the compact bone and partly also on the surface of the implant point to increased risks of immediate load of the blade implant. Stress distribution on the blade portion of the implant and the adjacent sponge bone is minimal. The broad application of blade implants presents a suitable non-conventional solution thank to shape optimisation.
