Introduction: This paper presents basic possibilities of surgical treatment of cystic lesions (cysts) in the maxillofacial region starting with decompression, marsupialisation, up to enucleation.

Methods: The authors summarize the results of individual therapeutic approaches used at the Department of Maxillofacial Surgery of F. D. Roosevelt University Hospital in Banská Bystrica. The paper presents a group of patients aged eighteen plus, who were hospitalized and surgically treated at our department with a diagnosis of cystic lesion in the upper jaw during one calendar year.

Results: The group of 32 patients consisted of 17 women and 15 men treated at our department during one year. A cystostomy was performed in three patients and a cystectomy in 29 cases. In 29 patients, the histological finding confirmed an odontogenic cyst, in two cases a nonodontogenic cyst and in one case an ameloblastoma.

Conclusion: When choosing the most appropriate type of surgery of the cystic lesion in the maxillofacial area, we take into consideration the histological type of the lesion, its anatomical location, age of the patient, presence of vital teeth in the lesion area, and the patient's collaboration. It is also necessary to think about a possible combination of surgical procedures such as delayed cystectomy, which is an effective method, especially in case of large cystic lesions.
